History: This patient had this ulcer on his leg for 15 years. He had seen over 50 physicians and had undergone several skin grafts without success.

Problem: Incompetent subfascial perforator veins causing superficial venous hypertension.

Treatment: Subfascial Endoscopic Perforator Surgery.

Comment: The ulcer began to heal almost immediately after the SEPS procedure. The healing was complete in three months.
